Description:
FDP has been in existence since March 1995. The Plan was created as a means to provide dental services for low-income Utahns. Presently, seven fixed clinics throughout Utah provide an extensive range of services to recipients of Medicaid [Traditional], Children's Health Insurance Plan [CHIP], and Primary Care Network [PCN] patients who qualify for those benefits. Monday-Thursday 7:30a-6:00p
