Description:
Provides outreach services and referrals for basic necessities food, clothing, shelter for the underserved, high risk individuals and families who face socio-economic, cultural and language barriers.
Programs to assist persons in becoming fuller participants in the community include: ESL; Afterschool and Summer Programs in Park City and Wendover; Prenatal, health, safety and budgeting classes in Spanish; immigration legal services; Bi-lingual counseling services for victims of domestic violence and depression.

History:
HCM began in 1994 to provide services for the poor and the underserved. The Promotora (Outreach Program) provides case management services and connects persons to existing services and resources.
