The mission of Mountainlands Community Health Center (MCHC) is to provide and collaborate with other partners to assure high quality health care for everyone in our community - especially those who experience economic, geographic, cultural, and language barriers to receiving care - regardless of their ability to pay.
Mountainlands Community Health Center strives to eliminate all barriers to receiving appropriate, quality health care, dental care and mental health care. Our doors are open to anyone. Qualifying individuals and families may receive a discount based on their family size and income. Eighty-five percent of our staff is Spanish-English bilingual and we offer culturally appropriate health services. Mountainlands offers case management for diabetic patients and patients who have cardiovascular disease. Mountainlands also provides discounted quality prenatal care.
Description:
Currently, Mountainlands Community Health Center serves 10,115 patients through over 36,035 patient encounters. Of those patients, 68% earn less than 100% of the Federal Poverty Guidelines and 73% of all patients are uninsured. Obstetrics represents 18% of the practice and pediatrics comprises 32%. Our patient population includes migrant and seasonal farm workers, homeless, and HIV positive individuals. MCHC patients are ethnically divers with over 65% speaking English as a second language. Seventy-eight percent of MCHC patients are Hispanic/Latino with Pacific Islanders, Asians, and Native Americans also being represented.
Utah County is one of the fastest growing metropolitan areas not only in the state of Utah but also in the nation with a (57%) growth increase during the past 15 years and a current population of 464,760. Currently, as many as 72,038 (15.5%) Utah County residents are estimated to be uninsured with 60,419 (13%) of the residents classified as under 100% of the Federal Poverty Guidelines. Utah County has been designated as a Health Provider Shortage Area (HPSA) serving a Medically Underserved Population (MUP) in family practice, dental and mental health services. While there has been some progress in the private sector in recent years in providing care for the vulnerable, particularly with the advent of Medicaid managed care and PCN; many Utah County providers do not see, or see very limited numbers of patients on a discounted basis.
Among our current staff are four Family Practice Physicians, three certified Physician’s Assistants, two Nurse Practitioners, one Clinical Psychologist (Ph.D.), and two Dentists. We are able to offer a medical home with continuity of care and a full array of family practice services including treatment for acute and chronic illnesses, access to low cost pharmaceuticals, mental health counseling, dental health, case management, obstetrics, gynecology, prenatal/perinatal, cancer and diabetes screenings, pediatrics, “enabling services,” inpatient hospital care, after hours coverage, and preventive services.MCHC offers high quality, culturally sensitive primary care, 85% of our staff are Spanish/English bilingual, five days a week including evenings and does so in an affordable manner for community residents who might otherwise delay or forgo health care or seek more costly services through area hospitals and emergency rooms.
History:
MCHC was established in May 1992 with initial funding from Intermountain Healthcare. Federal grant dollars (Section 330 Community Health Center) were added the following year. MCHC was managed by Salt Lake Community Health Centers, Inc., until January 1, 1995 whereupon MCHC became an independent, not-for-profit 501(c)(3) with a governing board of over 50% consumers. MCHC currently has two sites - the primary site, located in Provo - Mountainlands Community Health Center - and a new satellite site, located in Payson - Payson Family Health Center. Federal 330(e) Expanded Medical Capacity grant dollars were added June 1, 2007 to help fund the Payson satellite site.
