| Eligibility Requirements: |

Records from 1952 to the present are available for people who were born in the State of Utah. Death certificates are available from 1995 to the present for people born in Wasatch County or August 2006 to the present in the State of Utah.
Additional years back to 1905 are available for an additional expedite fee of $10.00. |

Simply download an application on the Wasatch County Health Department website, www.co.wasatch.ut.us/health, for a Certified Copy of a Birth or Death Certificate, fill out the application and bring it with photo ID that has your signature to the Heath Department or *mail it to us at 55 South 500 East, Heber City, Utah, 84032. *If mailing, a copy of the front and back of your photo ID is required. |
