| Eligibility Requirements: |

In order to qualify for the Wasatch County WIC Program you must live in Wasatch County, meet Federal income guidelines, be a woman who is pregnant, breastfeeding, or just had a baby OR be a child under 5 years of age (including foster children) have a nutrition risk as determined by a WIC professional. |

Contact your local WIC office located at 55 South 500 East, Heber City, 435-657-3266 for an appointment
You will need to bring the following items to your WIC appointment:
- Proof of income; this could be a month's check stubs, a bank statement, a note from an employer, a current Medicaid card, or a Family Employment Program or Food Stamp eligibility notice.
- Proof of Identification; you may bring a birth certificate, Social Security card, photo ID, Medicaid card, hospital bracelet or crib card, pay stubs or a utility bill.
- Proof of Residency; This could be a current bill, a Medicaid card, a rent/mortgage receipt, check stubs, or a forwarded piece of mail.
