CHIP is a state health insurance plan for children. Depending on income and family size, working Utah families who do not have other health insurance may qualify for CHIP. |

Children must fit the following requirements to be eligible for the insurance:
1. Family must meet the income guidelines
2. Under 19 years of age
3. Not currently covered by health insurance

| Intake Procedure: |
|
In order to sign up for CHIP, an application must be filled out. The application can be obtained in three ways:
1. Fill out and submit online at utahchip.org
2. Print out online at utahchip.org and mail into the address at the top of the application
3. Call 1-877-KIDS-NOW and have them mail you a copy of the application, which will be sent back to the address at the top of the application once it has been filled out.
Those who wish to fill out the application should gather the following information before applying in order to make the process smooth:
1. Social Security numbers for all children
2. Last three months pay stubs
3. Birth Certificates for each child |

Depending on your income level, you will either pay nothing, $30 per quarter, or $75 per quarter. You will not be asked to pay this premium until your second quarter with CHIP |
