| Eligibility Requirements: |

Call for an appointment. Urgent care is available and walk-in appointments are available on most days. Patients are required to fill out a registration form prior to seeing a provider, and each patient must provide proof of income every six months.
Patients payment responsibilities are based upon family size and family income. |

All patients must fill out a registration form, provide proof of income, e.g. one months worth of check stubs. |

For uninsured patients, all services are provided based on a sliding fee scale basis. Medicaid, Medicare, CHIP, PCN, and most private insurances are accepted. |
