Description : Provides free comprehensive care in pediatric orthopedics.
Hours of Operation: 8 am - 5 pm, M-F
Eligibility Requirements: Any child may be eligible for care at Shriners Hospitals if: * the child is under the age of 18, and * there is a reasonable possibility the child's condition can be helped Patients living outside of the Intermountain region should call our patient referral line at 1-800-237-5055.
Intake Procedure: Apply now for care or obtain a patient application through a local Shriners or Shrine Club;by writing to Shriners Hospitals, P.O. Box 31356, Tampa, FL 33631; or by calling 1-800-237-5055 (in Canada, please call 1-800-361-7256). Transportation to and from the hospital is the responsibility of the family. Often, local Shriners can help with transportation. The Shriners Hospital to which the child is being referred can assist with contacting local Shriners.
