| Intake Procedure: |
|
An initial intake is done with our billing representative during which necessary paperwork is completed including a "Consent to Treat" form and financial information. Following this initial intake, the Clinical Coordinator reviews the information and assigns a therapist to the case. |
| Insurance Information: |
|
Several grants are available to assist low income individuals and families, and eligibility is established during the initial intake. Those with insurance are also assisted in accessing these benefits. Additional funding opportunities such as Crime Victim Reparation and Ecclesiastical sources are also explored during the intake process. |

If you're approaching on I-15 from South, take the 800 North Orem Exit. Turn right from the offramp and immediately left at the light on 1200 West. Continue north for 4 1/2 blocks. The Center is on the left hand side of the road. Please enter on the south side of the building. If you're approaching on I-15 from North, take the 1600 North Orem Exit. Turn left from the offramp and travel east to the second light (1200 West, Orem). Turn right and continue south for 3 1/2 blocks. The Center is on the right. Please enter on the south side of the building.
