Agency:  Mountainlands Community Health Center
Program name: Health Services for Low-Income
Description : Provides primary medical, urgent care, comprehensive prenatal care and limited dental care for low-income and medically underserved populations provided at reduced costs based on income and family size. Mental health services are available but only through a referral by a primary care provider at Mountainlands.
Hours of Operation: 9 am - 9 pm, M-Th; 9 am - 5 pm, F
Eligibility Requirements: Unrestricted
Intake Procedure: New patients fill out registration form and must provide proof of income, e.g. one month's worth of check stubs, every six months.
Insurance Information: We accept most public and private insurances. We also accept the uninsured.
Languages Spoken: English, Spanish
Fees: Sliding fee based on income and family size
