Program name: Early Intervention Services
Description : Early Intervention Services are developmental services for children age birth to 3 years old who are experiencing a significant delay in development.

The professional staff at KOTM provide home visits and center-based activities to teach parents successful strategies to promote their child's development through every day activities. For children with hearing or vision impairments, KOTM and the Utah Schools for the Deaf and Blind (USDB) collaborate on providing appropriate services. Services are individualized to meet the unique needs of each child and family and parents are full partners in developing the service plan.
Hours of Operation: M-F: 8 am - 4 pm, some evening services available.
Eligibility Requirements: Children must be UNDER AGE 3 and reside in the KOTM service area (Alpine School District boundaries). Children with a qualifying diagnosis are automatically eligible for services. If the child has no diagnosis, the child is eligible if he/she has a moderate delay in just one area of development (communication, fine motor, gross motor, cognitive, social-emotional, and self-help skills). Anyone may call to make a referral, including parents. However, since services are voluntary, if you are not the parent, please check to see if the parent(s) are interested before contacting us. There is no charge for developmental evaluation, service coordination, or the development of the service plan (IFSP). For other services, there is a flat monthly fee determined by family income. Children who are eligible for Medicaid, WIC, and other similar programs pay no fee. Families may opt out of services at any time. We cannot list all qualifying diagnoses here due to space limitations, but some of the most common include Down Syndrome, Williams Syndrome, Prader-Willi Syndrome, Fetal Alcohol Syndrome, Fragile X Syndrome, and Cornelia de Lange Syndrome. Disorders such as Cerebral Palsy, Duchenne Type Muscular Dystrophy, various Myopathies, Tuberous Sclerosis, Arthrogryposis, congenital brain malformations, Pierre-Robin Sequence, and Spina Bifida are included. Pervasive Developmental Delay (PDD), Autism, and other autism spectrum disorders are included. Significant hearing and vision impairments and deafblind also qualify. Children with many metabolic conditions, such as Maple Syrup Urine Disease, Tay-Sachs, and Hunter Syndrome qualify. Children affected by congenital infections such as CMV, Herpes, HIV, or Syphillis are eligible. Children with Failure to Thrive are also eligible. Please call if your child's diagnosis is not listed but you are interested in services.
Intake Procedure: Upon referral, a KOTM Family Advocate will call the family to set up an appointment to evaluate the child's development and/or to document a qualifying diagnosis. <br><br> The Family Advocate explains the steps which lead up to the Service Plan (IFSP) meeting, reviews family income information, and tells the family what their monthly fee (if any) will be. At the Service Plan meeting, the family and KOTM team members determine the developmental goals for the child. Family goals may also be included. The team determines what services will be needed to help the family and child achieve the goals. The service plan is updated and changed as needed.
Insurance Information: Currently, NO third party insurance company is billed for EI services, with the exception of Medicaid.

Fees: Flat fee of $0 to $100 per month, based on family income; automatic fee exemption for enrollment in Medicaid, WIC and similar programs. Some services are not subject to the fee and are free to all eligible children.
