The Utah Telephone Assistance Program (UTAP) is designed to assist eligible households with monthly basic local telephone service. The program provides a monthly discount on the primary land line, and/or an installation discount for households with no telephone service. |

| Eligibility Requirements: |
|
Applicants must be at or below 135% of the federal poverty level or receive benefits from a social service such as Food Stamps, general assistance, Head Start, HEAT Program, Medicaid, National Free School Lunch Program, Public Housing Assistance, Refugee Assistance, Supplemental Security Income (SSI), Temporary Assistance for Needy Families, or Work Toward Employment assistance. |

There are four ways to apply for UTAP assistance.
1) Call the State of Utah UTAP Program at 1-800-948-7540.
2) Apply for UTAP at the same time as applying for the HEAT Program through Community Action Services.
3) Call the applicant's current telephone service and request an application to be mailed.
4) Visit http://community.utah.gov/heat to print an application. |
